At the epicenter of coordination is our case management team who work directly with physicians, families, and other community providers to ensure that all of a resident’s needs are being met.

Ongoing Case Management Services

Whether our case managers are troubleshooting a problem, working to secure additional services or medication changes for a resident, making emergent appointments and coordinating transportation, ordering personal care items, or just providing a listening ear, they help deliver coordinated care at every touchpoint - and help keep families informed about everything that is going on. Having this 24/7 care and coordination ensures that any issues are caught and addressed quickly so that residents can focus on the things that really matter.

Safety Checks Available Day and Night

Regular safety checks are part of our care planning process - whether it is two-hour safety and toileting checks - to more or less frequent visits, our staff continually looks in on your loved one to make sure they have everything they need.

24-Hour Supervision

We offer 24-hour supervision and assistance for all of our resident population. If you're seeking around the clock supervision for a loved one who is disoriented and is at risk for wandering, please consider a secure memory care wing such as the Champlain Valley Senior Community. Our secure care wings are under construction and are expected to be completed in 2025.
